Advance Health Care Directives (AHCD’s)
Every Christian Scientist should consider making an Advance Health Care Directive (AHCD) that specifies what type of health care you want, and names the individual(s) you would like to be able make health care decisions and to sign health care documents on your behalf, in case you aren’t able to make such decisions or sign such documents yourself.

If you do not already have a valid AHCD, please make one as soon as possible.  Broadview has a sample form here.  You may also obtain one from an attorney, law library, or on the Internet.  The document must be witnessed or notarized.

As part of the admissions process, you may be asked to sign a form stating that you would like to use your Medicare benefits for Christian Science nursing care at Broadview.  This form is required in order to receive Medicare coverage for your stay (if eligible) and the form must be notarized.  If you are not able to sign the form yourself, the person named in your AHCD may sign for you, and that’s one reason it’s so important to have the AHCD.

The Dominion Foundation – A benevolence fund that provides financial assistance to Christian Scientists who cannot pay the full cost of their care.

A benevolence fund that provides financial assistance to Christian Scientists who are training to become Christian Science nurses.
